Reablement

Reablement is a free short-term support service that helps you regain your skills, abilities and confidence to manage everyday tasks and live as independently as possible.

You can get reablement after an illness or injury that temporarily reduces your independence. It can also help with long-term and chronic conditions by showing you how to do things in a different way. Support that may help you achieve this could be:

Reablement focuses on identifying your strengths and using them to achieve personal goals that increase independence in your day-to-day life.

The service works closely with the NHS East London Foundation Trust (ELFT) to support your recovery after a visit to hospital. The trust’s community health team may recommend and request reablement support for you at home to assist your rehabilitation.

Reablement interventions usually last up to six weeks but can vary as they are tailored to your specific strengths and needs and reviewed on a regular basis.

An occupational therapist will schedule an assessment where you can discuss and agree with them on tasks specific to your support needs and goals.

Sometimes the reablement team may decide, after your assessment and interventions, that you have long-term care and support needs. If the national eligibility criteria are met in these cases, the social workers in the team will help you get the support you need.

If the assessment finds you are not eligible for support from us by law, we will still offer you information and advice to help you live independently and maintain a good quality of life.

Care Quality Commission (CQC) report

You have a right to know how care and support services are performing. All CQC registered providers of care and support are required to display their CQC ‘rating’ from when the service was last inspected.

The ratings are designed to improve transparency and provide you with a clear indication of the quality and safety of care and support provided.
