I have a support plan

Our locality teams deliver long-term assistance for your needs and review your support plan after 12 months.  

They will schedule a review meeting with you. If your needs or circumstances change you can contact us to ask for a review of your needs and support plan. Each locality team covers one (but can cover more) of the postcodes in Tower Hamlets: E1, E2, E3 and E14. Your locality team contact details are at the end of your support plan.

A friend, a relative or independent advocate (free service) can support you during the review process. 

The social care practitioner will review what is going well, what you would like to change, and your priorities since your last assessment or review. They will focus on using your strengths to continue or improve your current level of independence. You will:

The review will also decide if the national eligibility criteria to receive care and support are still being met.

To do this, locality teams collaborate with agencies including:

  • your landlord or housing association
  • your GP, nurse or any other health professional involved and 
  • community organisations that can provide support.
