CLDS deliver long-term assistance for your needs and review your support plan after 12 months.

The process

They will schedule a review meeting with you. But if your needs or circumstances change you can contact us to ask for a review of your support plan.

A friend, relative or an independent advocate (free service) can support you during the review process.

A CLDS practitioner will review what is going well, what you would like to change, and your priorities since your last assessment or review. They will focus on using your strengths to continue to improve your current level of independence. You will:

The review will also decide if the national eligibility criteria to receive care and support is still being met.

Your new support plan will reflect changes to your needs and personal budget.
