How to get support from CLDS

The Community Learning Disability Service (CLDS) accepts referrals from professionals as well as self-referrals from residents. If you suspect you or the person you are referring has an undiagnosed learning disability, complete our referral form. This will help you and CLDS decide whether an assessment is required.

First contact with CLDS

When CLDS receive a referral form indicating you or the person you are referring may have a Global Learning Disability they will screen and triage the referral within 48 hours and if there is evidence of a learning disability will arrange a ‘single assessment’ to confirm this. Occasionally, a single assessment may not reach a conclusion and further assessments and tests may be carried out if necessary.

CLDS will complete a Care Act 2014 assessment at the same time to fully understand your social care needs, as well as any other assessments or tests required.

If you have a Global Learning Disability, CLDS will tell you which of the three teams below  will provide you with support specific to your needs:

  • Transition team
  • Community health and wellbeing team
  • Mental health and behaviour support team

If the assessments confirm you do not have a Global Learning Disability, CLDS will provide  information and advice about available support. They may also direct you to mainstream adult social care if they determine there are eligible care and support needs.
