Community Heart Health Checks

Tower Hamlets Council, in partnership with Tower Hamlets GP Care Group, is offering free health checks to support the well-being of our local communities.

Our quick and friendly 15-minute health check helps individuals understand their health and take steps to reduce the risk of conditions like high blood pressure and diabetes. These checks are delivered by trained healthcare assistants and provide tailored health advice, signposting, and support.

Why Community Health Checks Matter

Bringing health checks into community spaces—such as local events, faith groups, and workplaces—makes it easier for people to access health support in a familiar and comfortable setting.

For Individuals:

  • Take charge of your health with expert guidance
  • Learn simple ways to improve your well-being
  • Reduce the risk of preventable conditions

For Communities:

  • Healthier, more informed residents
  • Less pressure on local health services
  • Stronger community connections through shared health initiatives

What’s Included in a health check?

  • Blood pressure check
  • Body Mass Index (BMI) & waist measurement
  • Type 2 diabetes risk score
  • Smoking & alcohol risk assessment
  • Personalised advice & signposting to support services

Let’s Work Together!

We’d love to explore ways to collaborate, whether by:

Providing free health checks at your community health and well-being events
Joining your existing outreach activities to offer health checks alongside your services
Partnering on tailored health initiatives that support residents’ well-being

If you have an idea for a partnership or would like to discuss how we can work together, get in touch!

Community health checks differ from the full NHS Health Check for individuals aged 40 to 74. If you're in this age group, you can find more details on the NHS website.

If an individual is registered with a GP in Tower Hamlets, their health check results can be shared with their GP practice, but only with their consent.
