Safeguarding adults reviews

The Tower Hamlets Safeguarding adults review (SAR) protocol outlines the agreed process for commissioning and learning from SARs. The key purpose for undertaking a SAR, previously known as a Serious Case Review, is so lessons can be learned from serious injury or death of an "adult at risk". 

These lessons can be shared widely so we reach an understanding of what needs to change to reduce the risk of further tragedies.

The cases of Mrs. N and Mrs. O were merged into a single thematic review that looks at their circumstances in relation to the standards of care and assistance they received. It was commissioned in March 2020 by the Tower Hamlets Safeguarding Adults Board.

The Mornington Hall and Bakers Court safeguarding adults review was undertaken due to concerns about the quality of care of six residents in two care homes based in Newham. A safeguarding adults review was commissioned by the Newham Safeguarding Adults Board.

A Tower Hamlets resident Ms R, a 92-year-old British White woman, was admitted to Bakers Court after a hospital admission and her care was looked at as part of the independent review. This explains the Tower Hamlets role in the safeguarding adult review.

Any individual (member of the public or professional) or agency may refer a case to be considered for a SAR using the SAR referral form.

Each summer, the Tower Hamlets Safeguarding Adults Board holds an annual conference that focuses on key safeguarding themes that staff from partner organisations across Tower Hamlets are invited to attend.
